Story
The request form should display both for logged in and logged out users. For users who are logged out, the form and button should be disabled.

Expected Behavior Before Changes
Before this PR, a user clicked on a ware to make a request, and they would end up at a page with just a notice that said "Unauthorized"
Expected Behavior After Changes
After this PR, when a logged out user clicks on a ware to make a request, they are able to see the form to make the request, but it is disabled. They can also see a notice at the top of the form prompting them to log in if they would like to fill it out.
If the user is logged in, they should still be able to fill out the form and submit requests.
